The number of service contracts and allocated funding that the Ministry of Health and local DHBs hold or provide for residential home care and home support services;
The names of the companies that currently receive funding or provide these residential home care and home support services contracts from/to the MoH or local DHB
The scope of the contracts and funds received (who are the services provided to and where, and the general nature of the services delivered);
The approximate annual value of the funding and cost of each of these contracts;
The expiry date of each of these contracts and the years on which the various funding is allocated/received; and
Primary contact point within the DHB or MOH or funder for each contract
